I installed and have been using PdfCreator for years. Now it stopped working. When I print a Print dialog opens but the PdfCreator Save dialog does not open. My PdfCreator\Spool directory is open. I can't reinstall because my antivirus keeps quarantining the setup file.
does the PDFCreator UI still open if you run the PDFCretor.exe manually?

Yes, the PDFCreator UI does open and I can print a test page from the Options dialog. But if I print a test page from the Windows Printer dialog the test page sits in the printer queue and does not open the PDFCreator Save dialog.
the PDFCreator test page doesn't actually get spooled, so the issue must be somewhere within the printing/spooling part. Do *ps and *inf files get created in your PDFCreator\spool directory when you print something to the PDFCreator printer? Do your user and the local system account have read/write accees there?
