How to include bookmarks

I downloaded and installed PDFCreator 1.7.3.  What are the steps for how I can include my Microsoft Word bookmarks (like Adobe Acrobat) within the PDF that is created.  I couldn't locate any info on how to do this from your site.

Same question. 9 years later. They haven't added option to add Word docx TOC into PDF bookmarks?
....Is this rocket science?
