Once converted to pdf format, has anyone gone back in and make comments and/or notes in the margins of the doc?

You're talking about editing a file, which is something I want to do too - add a comment etc.
I don't think it is supported yet but I saw a post on the blog about "PDF Architect" in development, and I assume that will allow editing.
I just want to add an "approved" stamp to the pdf.
